If your evaluation is related to litigation, please have your attorney contact us.
If your evaluation is for clinical purposes, a referral from your medical provider is required.
For all forensic evaluations, we prefer initial contact with attorneys/paralegals or the court.
For employment evaluations, we prefer initial contact with the employer.

Notice of Access to Evaluation Records & Consumer Rights
(Texas Health & Safety Code §181.105)
Services:
Price Proctor provides forensic psychological/neuropsychological evaluations.
Access to Records:
Because Price Proctor provides forensic services rather than health care, record requests are handled through the appropriate legal authority, such as the Court or attorney that referred you, rather than directly through our practice. This will be or would have been explained to you during the informed consent/notification process and documented in writing.
Any requests sent to the evaluating psychologist and/or Price Proctor will be referred to the Court or the attorney associated with your legal matter.
Complaints:
To file a complaint regarding the psychologist that examined you, you may do so with the Behavioral Health Executive Council (BHEC):
